FACILITY NEEDS: (You are responsible for setting up, tearing down and clean up for all tables and chairs.  We DO NOT provide linens, chair covers, decor (center pieces, etc.) plastic ware, paper plates, etc. All of these are to be provided by you.  Please do not assume because you see it, it may be used. Only specific equipment may be used during rental.)

I am submitting my donation deposit of ($100) and will submit the remainder of my payment upon approval of the event. (If event is not approved, deposit will be refunded.) Failing to comply with these standards will forfeit the rental.
